It is designed to connect veterans medical files with Defense Department files, providing a single, lifelong record for troops past their military separation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA officials have touted the ambitious project as transformational for veterans care, allowing veterans to access their complete medical history at any appointment, either within federal health care systems or, eventually, at private sector offices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But the size and scope of the work have raised numerous concerns in recent years.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/2019\/02\/05\/senators-skeptical-over-long-term-va-health-records-overhaul\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/3975154.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Senators skeptical over long-term VA health records overhaul<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The $16-billion, 10-year plan is designed to bring VA medical records in line with Defense Department files.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">On Thursday, officials from the <a href=\"https:\/\/www.gao.gov\/assets\/720\/712368.pdf\" target=_blank>Government Accountability Office<\/a> said that as of last fall, \u201cVA was at risk of deploying a system that did not perform as intended and could negatively impact the likelihood of its successful adoption\u201d due to lingering issues with the new records system.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Investigators found more than 360 \u201chigh-severity test findings\u201d after the new system was deployed at test sites last fall, but most were settled within a few months. Of the remaining issues, VA officials said they have developed fixes for most and corrections for the rest in later system updates.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Still, officials from the watchdog agency said that VA leaders should \u201cpostpone deployment of the new EHR in new locations\u201d until all known issues are resolved. VA officials downplayed that assessment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe GAO acknowledged the progress made since the last report \u2015 proof that we can continue to move forward with deployment activity while solving identified issues at the same time,\u201d Paula Paige, spokeswoman for VA\u2019s Office of Electronic Health Record Modernization, said in a statement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe fact that we aren\u2019t surprised by any of the issues in the report validates VA is tracking the right things.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The new records system has already been deployed at six sites in Nevada and Washington with \u201cno patient safety issues\u201d related to missing information or difficulties accessing files, according to the department. We are confident we are on track to resolve these issues as we continue preparing for our next deployment.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Lawmakers have vowed to keep close watch on that progress. During VA Secretary Denis McDonough\u2019s confirmation hearing last month, Senate Veterans\u2019 Affairs Committee Chairman Jon Tester, D-Mont., expressed concerns that the larger effort to reform VA\u2019s records systems \u201chas taken way too long and cost taxpayers way too much.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA officials have already moved more than 24 million veteran health records into the new system.
