{"id":78276,"date":"2024-05-23T12:00:00","date_gmt":"2024-05-23T12:00:00","guid":{"rendered":"https:\/\/one.sightlinemg.com\/airforcetimes\/uncategorized\/2024\/05\/23\/veteran-suicide-prevention-algorithm-favors-men-investigation-finds\/"},"modified":"2026-08-08T16:24:08","modified_gmt":"2026-08-08T16:24:08","slug":"veteran-suicide-prevention-algorithm-favors-men-investigation-finds","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/airforcetimes\/veterans\/2024\/05\/23\/veteran-suicide-prevention-algorithm-favors-men-investigation-finds\/","title":{"rendered":"Veteran suicide prevention algorithm favors men, investigation finds"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">An artificial intelligence program designed to prevent suicide among U.S. military veterans prioritizes white men and ignores survivors of sexual violence, which affects a far greater percentage of women, an investigation by The Fuller Project has found.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The algorithm, which the Department of Veterans Affairs uses to <a href=\"https:\/\/www.research.va.gov\/naii\/ai-inventory.cfm\" target=\"_blank\">target assistance to patients<\/a> \u201cwith the highest statistical risk for suicide,\u201d considers 61 variables, documents show, and <a href=\"https:\/\/www.documentcloud.org\/documents\/24679669-3527-notes\" target=\"_blank\">gives preference<\/a> to veterans who are \u201cdivorced and male\u201d and \u201cwidowed and male,\u201d but not to any group of female veterans.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Military sexual trauma and intimate partner violence \u2014 both <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35730002\/\" target=\"_blank\">linked to elevated suicide risk<\/a> among female veterans \u2014 are not taken into account.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Recently released <a href=\"https:\/\/www.mentalhealth.va.gov\/docs\/data-sheets\/2023\/2023-National-Veteran-Suicide-Prevention-Annual-Report-FINAL-508.pdf\" target=\"_blank\">government data show a 24% rise<\/a> in the suicide rate among female veterans between 2020 and 2021, four times the increase among male veterans during that one-year period. It was also 10 times greater than the 2.6% increase among women who never served in the military.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIt was difficult enough to be a woman in the military. We get harassed; we get bullied,\u201d said Paulette Yazzie, a 45-year-old Air Force veteran from the Navajo Nation. She served 13 years in the military, including a tour in Iraq.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cNow we\u2019re being pushed to the back \u2014 again,\u201d she said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yazzie, a former staff sergeant, teared up when informed that the VA\u2019s suicide prevention algorithm prioritized men. She said she faced constant sexual harassment and unwanted advances in Iraq and slept with the light on, the door locked, and a chair propped against it for extra protection during her deployment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThey always think about us second,\u201d she said. \u201cThis is going to cost people\u2019s lives.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"720\" height=\"480\" src=\"\/wp-content\/uploads\/2026\/08\/going-home.jpeg.jpg\" alt=\"\" class=\"wp-image-109996\" srcset=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/going-home.jpeg.jpg 720w, https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/going-home.jpeg.jpg?resize=300,200 300w\" sizes=\"auto, (max-width: 720px) 100vw, 720px\" \/><figcaption class=\"wp-element-caption\">Former Air Force Staff Sergeant Paulette Yazzie poses for a selfie during a 2012 deployment to Iraq. (Courtesy Paulette Yazzie)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The VA touts its machine learning model, REACH VET, \u201cas the nation\u2019s first clinical use of a validated algorithm to help identify suicide risk.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Launched in 2017, the system flags 6,700 veterans a month for extra help. VA officials say it produces a <a href=\"https:\/\/www.nimh.nih.gov\/news\/science-news\/2022\/study-shows-reach-vet-program-effective-for-veterans-at-high-risk-for-suicide#:~:text=The%20program%20is%20the%20nation's,local%20REACH%20VET%20program%20coordinators.\" target=\"_blank\">significant reduction<\/a> in suicide attempts by those veterans over the following six months.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On May 13, the agency\u2019s undersecretary for health, Carolyn Clancy, <a href=\"https:\/\/veterans.house.gov\/calendar\/eventsingle.aspx?EventID=6446\" target=\"_blank\">called the system<\/a> a \u201cgame changer\u201d during a congressional hearing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In an interview, Matthew Miller, the VA\u2019s executive director for suicide prevention, said the agency considered military sexual trauma, along with hundreds of other variables, as it crafted its algorithm. But officials decided to exclude a history of rape or assault from the model because it was not among \u201cthe most powerful for us to be able to predict suicide risk,\u201d he said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The algorithm identified being a man, especially a white man, as more predictive of suicide than clinical factors known to impact women, he said. Divorce and death of a spouse were predictive of suicide risk when the patient was male, Miller added, but not when she was female.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIt\u2019s offensive that VA could rationalize overlooking women veterans at risk of suicide,\u201d Allison Jaslow, chief executive of Iraq and Afghanistan Veterans of America, said of the algorithm. \u201cThis is a very serious issue.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Though female veterans are the VA\u2019s fastest-growing population, Jaslow said women are habitually ignored. She cited as an example the long, and ultimately successful, fight to get the VA to design prosthetics that fit women.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Meredith Broussard, research director for New York University\u2019s Alliance for Public Interest Technology, said she doubted the agency\u2019s AI system reached accurate conclusions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWhenever you have an algorithm that seems to favor the majority group \u2014 for example white men \u2014 and someone says, \u2018It\u2019s just math,\u2019 it\u2019s most likely the case where systemic bias is manifesting itself in the math,\u201d Broussard said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Though the suicide rate among male veterans remains higher than that for their female counterparts, the rate among female veterans is rising faster.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Joy Ilem, national legislative director for Disabled American Veterans, told The Fuller Project she was \u201cpuzzled\u201d by the VA\u2019s decision to exclude a long list of factors known to increase suicide risk among female veterans \u2014 including military sexual trauma, intimate partner violence, pregnancy, menopause, and firearm ownership.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ilem noted that the <a href=\"https:\/\/www.hsrd.research.va.gov\/publications\/forum\/spring18\/default.cfm?ForumMenu=Spring18-5\" target=\"_blank\">VA\u2019s own researchers have found<\/a> that risk factors for suicide are different for female and male veterans. Given the rising suicide rate among women, it makes sense to conclude that \u201csomething would be included that\u2019s more tailored,\u201d she said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The issue of algorithmic bias has gained traction in recent years. Both Presidents <a href=\"https:\/\/www.federalregister.gov\/documents\/2020\/12\/08\/2020-27065\/promoting-the-use-of-trustworthy-artificial-intelligence-in-the-federal-government\" target=\"_blank\">Donald Trump<\/a> and Joe Biden issued executive orders to promote transparency and accountability for AI products. The VA has identified <a href=\"https:\/\/www.research.va.gov\/naii\/ai-inventory.cfm\" target=\"_blank\">more than 100 programs<\/a> covered by those presidential decrees.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Last year, <a href=\"https:\/\/www.whitehouse.gov\/briefing-room\/presidential-actions\/2023\/10\/30\/executive-order-on-the-safe-secure-and-trustworthy-development-and-use-of-artificial-intelligence\/#:~:text=(d)%20Artificial%20Intelligence%20policies%20must,denied%20equal%20opportunity%20and%20justice.\" target=\"_blank\">Biden issued two orders<\/a> that require federal agencies to ensure AI models do not perpetuate discrimination.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cMy administration cannot \u2014 and will not \u2014 tolerate the use of AI to disadvantage those who are already too often denied equal opportunity and justice,\u201d he said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Miller, the VA\u2019s suicide prevention director, said he wasn\u2019t sure what the agency was doing to comply with the orders. A follow-up inquiry to the VA press office did not produce specifics.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe Office of Suicide Prevention is working with the VA AI lead and Chief Technology Officer (CTO) on the development of compliance reviews and monitors,\u201d the agency said in a written statement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The VA declined to provide data on veterans flagged by the REACH VET algorithm. The agency keeps a monthly dashboard tracking patients and outcomes, but an agency spokesperson said the VA would only share that information if it received a Freedom of Information Act request.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The VA is the nation\u2019s largest integrated health care system, serving 9 million people across 172 hospitals and more than 1,000 clinics. It operates in all 50 states, Puerto Rico, Guam, the U.S. Virgin Islands and Washington, D.C.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The number of women using VA services has quintupled since 2001, growing from 159,810 then to over 800,000 today.
