{"id":18157,"date":"2022-06-23T18:27:56","date_gmt":"2022-06-23T18:27:56","guid":{"rendered":"https:\/\/one.sightlinemg.com\/armytimes\/uncategorized\/2022\/06\/23\/setbacks-with-new-health-records-system-have-shaken-va-secretary\/"},"modified":"2026-08-08T05:47:08","modified_gmt":"2026-08-08T05:47:08","slug":"setbacks-with-new-health-records-system-have-shaken-va-secretary","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/armytimes\/veterans\/2022\/06\/23\/setbacks-with-new-health-records-system-have-shaken-va-secretary\/","title":{"rendered":"Setbacks with new health records system have \u2018shaken\u2019 VA secretary"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Veterans Affairs Secretary Denis McDonough acknowledged that his confidence in the department\u2019s<a href=\"https:\/\/www.militarytimes.com\/veterans\/2022\/04\/26\/lawmakers-urge-pause-in-new-va-health-records-system-rollout-amid-more-problems\/\" target=\"_blank\"> new $16 billion electronic medical records system<\/a> \u201chas been shaken\u201d by more setbacks, including new evidence that the changeover has resulted in dangerous medical complications for some patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI do now know there were instances of patient harm,\u201d McDonough said at a press conference on Wednesday. \u201cThere could be a range of factors that contribute to that, but the patient safety team here at VA can\u2019t rule out that the electronic health records have played a role in that.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">McDonough\u2019s comments came less than a week after department officials announced t<a href=\"https:\/\/www.militarytimes.com\/veterans\/2022\/06\/18\/va-to-delay-electronic-medical-records-deployment-to-additional-sites-until-2023\/\" target=\"_blank\">hey would delay deployment<\/a> of the Oracle Cerner Millennium software to new sites in Oregon and Washington until 2023, and push back deployment to medical offices in Idaho until next month.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Officials said those moves would \u201callow Oracle Cerner to put important system enhancements in place and make the necessary improvements to ensure system stability \u2026 as well as fix outstanding issues to address research workflow challenges.\u201d<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/pentagon-congress\/2022\/06\/18\/va-to-delay-electronic-medical-records-deployment-to-additional-sites-until-2023\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/3139743.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">VA to delay electronic medical records deployment to additional sites until 2023<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">VA sites in Portland and Seattle were scheduled to start work with the new system later this year.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">But a day later, <a href=\"https:\/\/www.spokesman.com\/stories\/2022\/jun\/19\/watchdog-reveals-flaw-in-cerner-computer-system-ca\/\" target=\"_blank\">the Spokesman-Review reported <\/a>that a draft inspector general report had found at least 148 veterans were harmed by medical record mistakes and shortfalls after the system was implemented at initial sites in Washington state last year.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The newspaper said that the system failed to deliver more than 11,000 orders for specialty care, lab work and other services, all without alerting health care providers the orders had been lost.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">McDonough acknowledged the report is being finalized and would not directly comment on the inspector general\u2019s findings.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But he did acknowledge for the first time that the records system caused significant problems in veterans\u2019 care, a line that in the past he has said would lead him to halt deployment in the interest of patient safety.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The 10-year, $16 billion records overhaul was put in motion by President Donald Trump and was meant to bring veterans\u2019 health records in line with military files. In the past, the two agencies used separate records systems with intermediary programs to transfer data between them, causing information issues for patients and physicians.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/management\/pay-benefits\/2022\/06\/22\/defense-electronic-health-records-rollout-is-halfway-done-even-as-vas-is-delayed\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"215\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Electronic-health-records-DVIDS.6595521.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Defense electronic health records rollout is halfway done, even as VA\u2019s is delayed<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">DoD remains on schedule to deploy its Genesis electronic health records system worldwide by the end of 2023.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">Pushing new sites back to 2023 is the latest in several delays in the deployment schedule, including a full stop of the program in summer 2021 to evaluate other training and operation concerns raised by the inspector general.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">McDonough deflected questions about what VA will do if the records system cannot meet department standards for accuracy and reliability.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cRight now, we\u2019re trying our best to make this Cerner option work,\u201d he said. \u201cBecause we all believe the best thing for our patients\u2019 outcome, for patient safety and for our clinicians\u2019 practice is a well-functioning electronic health care record.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Earlier this week, officials from Oracle Cerner released a statement saying they are making technical changes in the system \u201cwith an emphasis on patient safety, to ensure the system exceeds the expectations of providers, patients, and the VA.\u201d They also promised to put \u201csubstantially more resources to this program\u201d in coming months.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But lawmakers have grown increasingly frustrated with the setbacks and growing reports of potential medical complications caused by the new system.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/pentagon-congress\/2022\/04\/26\/lawmakers-urge-pause-in-new-va-health-records-system-rollout-amid-more-problems\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/fed-nicole-data-technologiejpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Lawmakers urge pause in new VA health records system rollout amid more problems<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The VA hopes to deploy the new system to multiple new sites later this year.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">On Wednesday, before McDonough\u2019s press conference, House Veterans\u2019 Affairs Committee Chairman Mark Takano, D-Calif., and the panel\u2019s technology panel chairman Frank Mrvan, D-Ind., called the draft reports \u201cseriously troubling,\u201d especially in light of upbeat assessments about the records system delivered to Congress by VA officials in recent months.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe have already begun discussions with VA on the performance of Cerner and requested an official briefing on the forthcoming report,\u201d the pair said in a statement. \u201cOnce released, we will be reviewing the findings closely in order to determine if there are any contractual or legal repercussions of these [inspector general] draft findings.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA officials said despite the controversies, they still plan to begin use of the new records system at the Boise VA Medical Center on July 23.
