But we don&#039;t know definitively,&quot; said Hawaii&#039;s top Navy doctor.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.navytimes.com\/news\/your-military\/2022\/03\/30\/dod-wants-another-1-billion-to-fix-hawaii-water-crisis\/\" target=\"_blank\">After months of resistance<\/a>, the military in April agreed to an order from the state of Hawaii to drain the tanks and close the Red Hill facility. A separate report that the Defense Department provided to the state Department of Health on Thursday said the earliest it could defuel the tanks safely is December 2024.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigation report listed a cascading series of mistakes, beginning in May 6, 2021, when operator error caused a pipe to rupture and 21,000 gallons of fuel to spill when it was being transferred between tanks. Most of this fuel spilled into a fire suppression line and sat there for six months, causing the line to sag. A cart rammed into this sagging line on Nov. 20, releasing 20,000 gallons of fuel.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe lack of critical thinking, intellectual rigor, and self-assessment by key leaders at decisive moments exemplified a culture of complacency and demonstrated a lack of professionalism that is demanded by the high consequence nature of fuel operations,\u201d the investigation states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The area where the cart hit the line isn\u2019t supposed to have fuel, and so the officials who responded to the spill didn\u2019t have the right equipment to capture the liquid.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe team incorrectly assumes that all of the fuel has been sopped up,\u201d Adm. Sam Paparo, the commander of the U.S. Pacific Fleet, told reporters at a news conference. About 5,000 gallons wasn\u2019t recovered.<\/p>\n\n\n\n\n\n<p class=\"wp-block-paragraph\">\u201cMeanwhile, over the course of eight days, that fuel enters into this French drain that is under the concrete and seeps slowly and quietly into the Red Hill well. They have their whole lives ahead of them,\u201d says Army major.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">Red Hill officials thought that only 1,618 gallons had leaked in the May spill and that they recovered all but 38 gallons. They noticed that one of the tanks was short 20,000 gallons, but believed it had flowed through the pipes and didn\u2019t realize it had flown into the fire suppression line. They didn\u2019t report the discrepancy to senior leadership.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">After the November spill when people started getting sick, the military moved about 4,000 mostly military families into hotels for months while they waited for their water to be safe again.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The spill contaminated the Navy\u2019s water system. Fuel didn\u2019t get into the Honolulu municipal water supply. But concerns the oil might migrate through the aquifer and get into the city\u2019s wells prompted the Honolulu Board of Water Supply in December to shut down a key well serving some 400,000 people. The agency has been asking residents to conserve water because of this and unusually dry weather.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report said officials <a href=\"https:\/\/www.navytimes.com\/news\/your-military\/2021\/12\/07\/i-regret-i-did-not-tell-our-families-not-to-drink-the-water-hawaii-joint-base-commander-says\/\" target=\"_blank\">defaulted to assuming the best<\/a> about what was happening when the spills occurred, instead of assuming the worst, and this contributed to their overlooking the severity of situation.<\/p>\n\n\n\n\n\n<p class=\"wp-block-paragraph\">Paparo said the Navy is trying to move away from that. He called it an ongoing process \u201cto get real with ourselves\u201d and \u201cbeing honest about our deficiencies.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">He recommended that the Navy review operations at 48 defense fuel storage facilities worldwide.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe cannot assume Red Hill represents an outlier, and similar problems may exist at other locations,\u201d Paparo wrote in the report.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/navytimes\/news\/your-navy\/2021\/12\/12\/toxic-water-consumes-daily-lives-of-many-hawaii-military-families-with-no-end-in-sight\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"157\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/stir.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Toxic water consumes daily lives of many Hawaii military families, with no end in sight<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">Nothing can alleviate &#039;the complete mental load that we\u2019re all bearing right now.&#039;<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">While the Navy did not release its plan for defueling the complex, Hawaii\u2019s Department of Health released it to the public Thursday evening \u201cin the interest of transparency,\u201d according to Deputy Director of Environmental Health Kathleen Ho.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cRed Hill needs to be shut down as quickly as possible and we fully expect that the Navy will marshal all possible available resources to defuel and decommission the facility,\u201d Ho said in a statement. \u201cHowever, with the extensive repairs needed and the Navy\u2019s history of spills from unsafe pipelines, our first priority continues to be ensuring that all defueling activities are performed safely.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Navy\u2019s defueling plan, a copy of which is available below, won\u2019t be completed until the end of 2024.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/s3.amazonaws.com\/static.militarytimes.com\/assets\/pdfs\/1656694818.pdf\" target=\"_blank\">The Navy\u2019s plan for defueling Red Hill<\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The vice chief of naval operations has assigned the head of U.S. Fleet Forces Command, Adm. Daryl Caudle, to determine disciplinary measures for those in uniform. Recommendations regarding civilian employees will be sent to their supervisors, Paparo said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report said the investigation revealed that poor training and supervision, ineffective leadership and an absence of ownership regarding operational safety also contributed to the incident.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In particular, the investigation highlighted a February 2021 decision by the commanding officer of Fleet Logistics Center Pearl Harbor to remove uniformed military oversight from day-to-day operations at Red Hill. The report said this significantly increased the risks of fuel handling operations.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It also noted key leaders at the scene of the November 2021 spill failed to exercise a sense of urgency, critical thinking, forceful backup and timely and effective communication demanded \u201cby the seriousness of the situation.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The head of U.S. Indo-Pacific Command, Adm. John Aquilino, told reporters Thursday that, despite the loss of the Red Hill facility as a fuel hub, \u201call missions are able to be executed through the means of where and how we place fuel reserves needed to support operational requirements.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To replace what Red Hill provided, Aquilino said, a \u201cbroad spectrum\u201d of distributed fuel hubs will replace the facility, including other existing facilities, new depots and \u201cpotentially afloat capabilities.\u201d<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/navytimes\/news\/your-navy\/2022\/03\/29\/how-shuttering-red-hill-could-make-fueling-the-fleet-more-complex\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/AP22061751982137.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">How shuttering Red Hill could make fueling the fleet more complex<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The Pentagon announced plans to close the massive Pearl Harbor facility after fuel leaked into tap water, affecting thousands of military families and threatening the civilian population&#039;s water supply.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">But while the Navy has repeatedly pointed to Red Hill as a vital part of U.S. national defense, yet it was left for decades without proper oversight. He said this shows a lack of leadership, investment and gross negligence, U.S. Rep. Kaiali\u2019i Kahele, D-Hawaii, said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe Navy\u2019s report states that the fuel leaks at Red Hill in May and November 2021 were preventable. This is shocking and deeply concerning,\u201d Kahele said in a statement.<\/p>\n\n\n\n\n\n<p class=\"wp-block-paragraph\">David Henkin, an attorney for Earthjustice, which has been filing legal challenges against the facility, said the Navy has failed to learn from its mistakes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cRather than act swiftly to remove the more than 100 million gallons of toxic fuel that remain perched over Oahu\u2019s sole source aquifer, the Navy proposes to take another two and a half years \u2014 until the end of 2024 \u2014 to defuel the Red Hill
