{"id":44508,"date":"2019-04-15T15:15:40","date_gmt":"2019-04-15T15:15:40","guid":{"rendered":"https:\/\/one.sightlinemg.com\/marinecorpstimes\/uncategorized\/2019\/04\/15\/heres-why-no-one-was-fired-after-va-mistakes-cost-a-veteran-his-foot\/"},"modified":"2026-08-08T08:17:52","modified_gmt":"2026-08-08T08:17:52","slug":"heres-why-no-one-was-fired-after-va-mistakes-cost-a-veteran-his-foot","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/marinecorpstimes\/veterans\/2019\/04\/15\/heres-why-no-one-was-fired-after-va-mistakes-cost-a-veteran-his-foot\/","title":{"rendered":"Here\u2019s why no one was fired after VA mistakes cost a veteran his foot"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Officials at the Indianapolis Veterans Affairs hospital insist they wanted to fire the administrator whose <a href=\"https:\/\/www.militarytimes.com\/news\/pentagon-congress\/2019\/04\/11\/va-administrative-mistakes-caused-one-veteran-to-lose-his-foot\/\" target=_blank>mistakes led to the amputation of a veteran\u2019s foot<\/a> due to missed home care appointments, but couldn\u2019t because the staffer retired <a href=\"https:\/\/www.militarytimes.com\/veterans\/2018\/01\/03\/va-firings-spiked-after-trump-signed-the-new-accountability-law-last-year\/\" target=_blank>before they could act<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cHad (the investigation) been completed before the employee retired, the Roudebush Veterans Affairs Medical Center would have proposed the employee for termination,\u201d Craig Larson, spokesman for the VA\u2019s Chicago District, said in a statement. \u201cThe employee chose to retire while the investigation was ongoing, and there was nothing the center could do to stop that.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Last week, officials from the <a href=\"https:\/\/osc.gov\/PublicFiles\/FY2019\/DI-18-0169%3b%200225%3b%200226\/DI-18-0169%3b%200225%3b%200226%20Redacted%20Agency%20Report.pdf\" target=_blank>U.S. Office of Special Counsel<\/a> said that an administrative decision in 2017 to stop recording home health care consults into a VA\u2019s patient record system jeopardized the health of numerous patients at the Indiana VA hospital, and forced at least one of them to lose his foot to a medical amputation.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/pentagon-congress\/2019\/04\/11\/va-administrative-mistakes-caused-one-veteran-to-lose-his-foot\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/AP_560842415588.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Vet gets his foot cut off because VA made administrative errors<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The problems arose because VA social workers stopped recording home health care consults into a VA\u2019s patient record system.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">That man, who had been discharged from care after a diabetes treatment, was left to change the dressings on his foot wound by himself for several days, even though VA staffers were supposed to do that.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201c[His] worsening infection \u2026 and subsequent amputation appears to have been related to the delay of the dressing changes by the home care agency,\u201d the report states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pete Scovill, public and congressional affairs officer for Veteran Health Indiana, said hospital officials \u201cremain in close contact with the veteran\u201d today and have offered an apology and \u201coptions moving forward.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">He also said that all affected staff have been re-trained to ensure that home health care consults are being properly conducted and recorded.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But the Special Counsel report noted that despite the grave nature of the mistakes, no staffers were fired. A social work assistant chief was reassigned to a different position, and the senior chief retired.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Larson defended the moves, saying center leaders took immediate action in response to the whistleblower allegations. But officials could not prevent the senior chief from retiring, and could not take any adverse job actions after that.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">President Donald Trump made accountability at VA a key promise during his election campaign. After less than six months in office, he signed a new department accountability measure into law, speeding up the time in which staffers can be fired and allowing the department to recoup bonuses from individuals later convicted of criminal wrongdoing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But VA officials said none of those measures would apply in this case.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/veterans\/2018\/07\/17\/va-union-leaders-spar-over-impact-of-new-accountability-law\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"225\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/IMG_8008.JPG.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">VA, union leaders spar over impact of new accountability law<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">Administration officials have repeatedly hailed the measure, but critics call it demoralizing and unfair to lower-level VA employees.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">Overall firings at the department have increased each of the last three calendar years, as the number of VA staff has also continued to climb.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In 2016, VA fired 2,001 individuals through regular removals and probationary terminations. In 2017, that number rose to 2,537.
