The collision rendered both aircraft uncontrollable, and both pilots ejected, landed safely and were recovered with only minor injuries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Both aircraft were destroyed when they impacted on the <a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/2016\/06\/10\/air-force-probing-crash-of-reaper-drone-in-nevada-desert\/\" title=\"\" class=\"\" target=\"_blank\">Nevada Test and Training Range<\/a>, 55 miles northwest of Nellis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This resulted in the loss of more than $30 million in aircraft and an environmental clean-up cost of $108,000, the report reads. <\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"980\" height=\"577\" src=\"\/wp-content\/uploads\/2026\/08\/Capture2.JPG_c44d99.jpg\" alt=\"\" class=\"wp-image-57798\" srcset=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Capture2.JPG_c44d99.jpg 980w, https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Capture2.JPG_c44d99.jpg?resize=300,177 300w, https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Capture2.JPG_c44d99.jpg?resize=768,452 768w\" sizes=\"auto, (max-width: 980px) 100vw, 980px\" \/><figcaption class=\"wp-element-caption\">Midair collision simulation between two A-10s over a Nevada test range in September 2017. (Air Force)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The crewmembers had been conducting close air support as part of the Weapons Instructor Course for one of the pilots. That pilot said he would maintain an altitude below 10,000 feet Mean Sea Level, while the other pilot would maintain the block of airspace between 11,000 and 12,000 feet. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigation into the incident found that both pilots arrived at their aircraft with all the appropriate equipment, including night vision goggles.<br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As the sortie progressed, however, increased radio communications, coordination with other participants, and tasks related to weapons delivery diverted the first pilot\u2019s time and attention away from effectively crosschecking his aircraft\u2019s altitude.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The pilot stated in the report that he didn\u2019t hear an audible notification indicating his aircraft had climbed above the directed altitude and into the buffer zone established to separate the two A-10s. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Between 7:28 p.m. and 7:43 p.m., the pilot progressively climbed 1,400 feet above his prescribed altitude. Bruce Munger, president of the Aircraft Accident Investigation Board. Munger wrote that a contributing factor to the incident was the breakdown of the pilot\u2019s visual scanning procedures. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe amount of time or attention spent performing a visual scan of aircraft instruments was not sufficient and did not allow [the pilot] to identify that an altitude deviation had occurred,\u201d Munger wrote. <br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Over-reliance on audible notification, as well as high task loading during the training scenario, likely caused the pilot to spend less time focusing
