Frank Mrvan, D-Ind., chairman of the House Veterans\u2019 Affairs Committee\u2019s technology panel. \u201cWe have seen no indication that the system is ready for the challenges at [bigger] facilities.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThese issues have to be fixed first.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Those issues for the 10-year, $16 billion health records modernization effort include multiple outages over the last six months, reports of a near-death at a Spokane VA medical center last month connected to the records turnover, and the potential of billions in cost overruns if the project is delayed even by a single year.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/pentagon-congress\/2022\/03\/07\/concerns-over-va-electronic-health-records-mount-after-another-system-problem\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Mann-Grandstaff-Department-of-Veterans-Affairs-Medical-Center-1.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Concerns over VA electronic health records mount after another system problem<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">A system issue forced the health records offline for a day last week, disrupting medical operations at a Washington state site.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">But VA officials insisted that the new records system \u2014 based on the Cerner Millennium software platform \u2014 is still fundamentally sound, and that new training put in place this spring has set up a more successful implementation of the software in years to come.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThis is one of the most complex clinical and business transformation endeavors in the department\u2019s history,\u201d said Dr. Terry Adirim, head of VA\u2019s Electronic Health Record Modernization Integration Office.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe are transitioning from the current nearly 40-year-old [records system] to a new single state-of-the-art system that requires a different way for personnel to do their work. I am sensitive to how hard this can be.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Last year, VA leaders paused the system rollout for six months amid a host of problems at the first deployment site, Mann-Grandstaff VA Medical Center in Spokane. Last month, that work resumed, with new training for staff, new management for the program and a new deployment at nearby department sites in Walla Walla.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, lawmakers on Tuesday said those changes have not erased their lingering concerns.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A <a href=\"https:\/\/www.spokesman.com\/stories\/2022\/apr\/24\/veteran-hospitalized-at-spokane-va-after-missing-h\/\" target=\"_blank\">report by the Spokesman Review this weekend<\/a> detailed how one veteran\u2019s medication history was halted because of the records turnover, eventually resulting in heart failure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA officials classified the incident as a mistake by staff, rather than an error prompted by the new records system.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI don\u2019t believe that there\u2019s any evidence that this system has harmed any patients, or that it will going forward,\u201d Adirim said. \u201cI think it\u2019s important that we have the processes in place to mitigate any problems that could come up due to human error.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA leaders also insist that the program is scheduled to be finished in six years, in keeping with the original 10-year schedule.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If not, it could mean a big financial expense for the government. An inspector general report released Monday said every additional year it takes to finish the project could cost $1.9 billion more above the project\u2019s $16 billion price tag.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On Tuesday, representatives from the VA Inspector General\u2019s office said serious concerns remain in how the project is being managed and rolled out.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/veterans\/2021\/12\/01\/va-to-resume-medical-records-overhaul-in-early-2022-with-new-leadership-guiding-the-work\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/1413724.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">VA to resume medical records overhaul in early 2022, with new leadership guiding the work<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The program has been on hold since the summer after reports of problems with the inital rollout last year.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">\u201cStaff have repeatedly found VA\u2019s estimates unreliable and incomplete,\u201d said David Case, deputy inspector general. \u201cThere is a lack of transparency due to inadequate reporting to Congress, stove-piped governance with decision-making \u2026 and deficient processes for transparently and promptly responding to concerns.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But Adirim insisted that the system has already been \u201csuccessfully implemented at more than 50% of the Department of Defense sites across the United States,\u201d suggesting those lingering problems can be overcome.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When the project was first announced by President Donald Trump in 2017, the idea was to put the Defense Department and VA on the same health records system for the first time. That would provide a single medical file that followed individuals from military enlistment into post-service life.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patrick Sargent, senior vice president of Cerner Government Services, called the project \u201ca once-in-a-lifetime opportunity to fundamentally change healthcare for generations to come\u201d during Tuesday\u2019s hearing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But lawmakers have voiced concerns about the cost and turmoil of reaching that goal.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI realize that VA needs a modernized records system,\u201d said veterans committee Chairman Mark Takano, D-Calif. &#8221; Continuing with [the old VA system] is not sustainable long term.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cHowever, we must make sure that we are not putting undue burden on our front-line workers or endangering veterans in the process.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In advance of the hearing, the Cerner system went offline for several hours on both Monday and Tuesday, disrupting some care at the sites already using the new software.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">VA officials plan to deploy the new system at sites in Columbus, Ohio, in coming days. VA has plans for further rollouts later in the year, but whether the objections by Congress on Tuesday will change that remains unclear.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The VA
