{"id":37139,"date":"2022-03-09T02:58:50","date_gmt":"2022-03-09T02:58:50","guid":{"rendered":"https:\/\/one.sightlinemg.com\/navytimes\/uncategorized\/2022\/03\/09\/ntsb-navy-contractors-helicopter-nose-dived-before-crash-killed-4\/"},"modified":"2026-08-08T19:38:03","modified_gmt":"2026-08-08T19:38:03","slug":"ntsb-navy-contractors-helicopter-nose-dived-before-crash-killed-4","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/navytimes\/news\/your-navy\/2022\/03\/09\/ntsb-navy-contractors-helicopter-nose-dived-before-crash-killed-4\/","title":{"rendered":"NTSB: Navy contractor\u2019s helicopter nose dived before crash killed 4"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">HONOLULU \u2014 The National Transportation Safety Board said Tuesday that a helicopter <a href=\"https:\/\/www.navytimes.com\/news\/your-military\/2022\/02\/22\/us-military-responding-to-helicopter-crash-in-hawaii\/\" target=\"_blank\">that crashed in Hawaii last month<\/a> and <a href=\"https:\/\/www.navytimes.com\/news\/your-navy\/2022\/02\/23\/navy-contractor-names-4-killed-in-hawaii-helicopter-crash\/\" target=\"_blank\">killed all four people on board<\/a> nose-dived from 200 feet (60 meters) above ground after it unexpectedly stopped while making a shallow left turn.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The&nbsp;agency\u2019s preliminary report&nbsp;on the Feb. 22 crash on Kauai said U.S. Navy contractor Croman Corp. was using the <a href=\"https:\/\/www.defensenews.com\/air\/2020\/07\/06\/lebanon-to-sell-outdated-aircraft-as-air-force-eyes-new-trainers\/\" target=\"_blank\">Sikorsky S-61N helicopter<\/a> to retrieve inert training torpedoes from the Pacific Ocean at the time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The crew\u2019s job involved locating a training torpedo in open waters and retrieving it using a recovery basket or cage. The helicopter then was expected to return the torpedo to the nearby <a href=\"https:\/\/www.navytimes.com\/news\/your-military\/2021\/05\/30\/military-weapon-system-test-fails-to-intercept-target\/\" target=\"_blank\">Pacific Missile Range Facility<\/a> using a sling load, which is cargo carried beneath a helicopter.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The agency said multiple witnesses reported the helicopter \u201cgradually pitched nose down and impacted nose first, in a near vertical attitude.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A fire then \u201cincinerated much of the helicopter\u2019s structure,\u201d the report said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The aircraft was returning to an ordnance recovery cage area when it crashed.<\/p>\n\n\n\n<p
