{"id":38473,"date":"2024-01-19T20:28:50","date_gmt":"2024-01-19T20:28:50","guid":{"rendered":"https:\/\/one.sightlinemg.com\/navytimes\/uncategorized\/2024\/01\/19\/misplaced-flashlight-in-f-35-engine-results-in-4-million-in-damage\/"},"modified":"2026-08-07T21:17:29","modified_gmt":"2026-08-07T21:17:29","slug":"misplaced-flashlight-in-f-35-engine-results-in-4-million-in-damage","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/navytimes\/news\/your-navy\/2024\/01\/19\/misplaced-flashlight-in-f-35-engine-results-in-4-million-in-damage\/","title":{"rendered":"Misplaced flashlight in F-35 engine results in $4 million in damage"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">A foreign object left inside an F-35 engine at Luke Air Force Base, Arizona, in early 2023 caused nearly $4 million in damages, a recent Air Force<a href=\"https:\/\/www.afjag.af.mil\/Portals\/77\/AIB-Reports\/2023\/15%20MAR%202023,%20AETC,%20F-35A,%20Luke%20AFB,%20AIB%20Report.pdf\" target=\"_blank\"> accident investigation<\/a> report revealed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The item \u2014 a handheld flashlight \u2014 was sucked into the engine\u2019s air intake during a maintenance ground run on the night of March 15, <a href=\"https:\/\/www.airandspaceforces.com\/new-report-flashlight-f-35-engine\/\" target=\"_blank\">Air &amp; Space Forces Magazine first reported<\/a>. The aircraft belonged to the 56th Fighter Wing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During an idling procedure, a team of three maintainers started the engine without issue before going through typical checks, including idling for five minutes to monitor for fuel leaks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It wasn\u2019t until the airmen turned off the engine that anything seemed amiss. During the shutdown, one maintainer noted \u201cabnormal noises.\u201d None of the airmen were injured.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One of the maintainers carried out a tool inventory check after installing a \u201cmetering plug into an engine fuel line,\u201d the investigation noted. The process took place before another maintainer used the flashlight to carry out a \u201cBefore Operations Servicing\u201d inspection, the report said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Investigators determined that the maintainers\u2019 lack of adherence to standard cautionary items highlighted in the Joint Technical Data checklist before the run contributed to their leaving the flashlight behind.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Investigators also found that \u201ccomplacency\u201d related to the F-35\u2032s Autonomic Logistics Information System, or ALIS, is partly to blame.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe substantial number of checklists and difficulty in accessing the correct ones causes complacency when users reference the required maintenance procedures,\u201d the report noted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ultimately, the mishap damaged the aircraft\u2019s second stage rotor, third stage rotor, fifth stage rotor, sixth stage rotor, fuel nozzle, bypass duct, high pressure compressor, high pressure turbine and fan inlet variable vane, the report found.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In total, the estimated cost of damages associated with the accident came in at $3,933,106.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report concluded that the inspecting maintainer \u201cfailed to clear the inlet of foreign objects after completing the required inspections for an engine run after they exited the aircraft inlet.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cFailure to complete checklist tasks resulted in a flashlight being left inside the inlet,\u201d the report
