Here\u2019s how the Air Force will make sure it doesn\u2019t happen again."},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">WASHINGTON \u2014 Sloppy maintenance work at an Air Force depot was at the root of a tragic <a href=\"https:\/\/www.marinecorpstimes.com\/2017\/07\/11\/15-marines-one-sailor-killed-in-the-corps-deadliest-crash-in-more-than-a-decade\/\" target=_blank>crash <\/a>of a Marine Corps Reserve KC-130T that resulted in<a href=\"https:\/\/www.marinecorpstimes.com\/military-honor\/2018\/07\/14\/one-year-after-deadly-kc130t-mississipp\" target=_blank> <\/a>the deaths of 16 service members in July 2017.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">According to a command investigation and about 2,000 pages of supporting documentation, <a href=\"https:\/\/www.militarytimes.com\/2018\/12\/05\/investigation-blames-air-force-and-navy-for-systemic-failures-in-fatal-marine-corps-c-130-crash-that-killed-16\/\" target=_blank>obtained exclusively by Defense News and Military Times<\/a>, a corroded blade broke off of the aircraft, sliced through the fuselage, and set off a chain of events that ended with the plane splitting into three pieces and crashing into a Mississippi soybean field.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The blade was last overhauled at Warner Robins Air Logistics Complex in 2011, where civilian maintainers are responsible for rooting out corrosion and other such problems. But although the investigators found evidence that small cracks and pits were already present in the propeller blade, maintainers did not properly treat it \u2014 allowing it to grow into a long fracture.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/news\/2018\/12\/05\/investigation-blames-air-force-and-navy-for-systemic-failures-in-fatal-marine-corps-c-130-crash-that-killed-16\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"189\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/KC-130T-crash-AP.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Investigation blames Air Force and Navy for systemic failures in fatal Marine Corps C-130 crash that killed 16<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">A corroded propeller was sent through an Air Force maintenance depot but was not repaired.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">Now, the Air Force is taking steps to make sure that those mistakes are never repeated.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;We have commissioned an independent review team made up of experts from the Navy, Marine Corps the Air Force and industry to look at the blade overhaul process, to revamp that process, to ensure that we have the proper procedures, the latest technology, the right technical manuals and the right training,\u201d Brig. Gen. (WLBT-TV via AP)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">The team has come up with a standardized overhaul process that can be applied to Air Force, Marine Corps and Navy propellers \u2014 a massive departure from the procedures used in 2011, which set special standards for Navy and Marine Corps blades.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/s3.amazonaws.com\/static.militarytimes.com\/assets\/public-release-command-investigation-report-yanky-72-redacted.pdf\" target=_blank><b>Read the Marine Corps investigation of the 2017 KC-130T mishap here.<\/b><\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">WR-ALC, like other Air Force repair facilities, threw out its records after two years, making it difficult to know exactly which maintenance requirements were skipped or overlooked. But investigators, who assessed the mishap aircraft\u2019s blades and the WR-ALC\u2019s workflow, made many troubling findings.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For instance, the investigation found no evidence that the corrosion-preventing permatreat coating or epoxy primer was ever put onto the blade that incited the accident, even though such treatments were required by the Navy at the time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Investigators also discovered that eddy current inspections, which use electromagnetic waves to detect anomalies under a metal surface, were also not performed at WR-ALC during 2011, even though they were a compulsory part of the Navy process.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/defensenews\/breaking-news\/2017\/07\/27\/marine-corps-kc-130ts-grounded-until-further-notice\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"200\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/1873168.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Marine Corps KC-130Ts grounded until further notice<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The Marine Corps has grounded its fleet of KC-130Ts \u201cout of an abundance of caution\u201d following the events of the deadly crash earlier this month, the service has confirmed to Defense News.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">WR-ALC <a href=\"https:\/\/www.navytimes.com\/news\/your-navy\/2018\/04\/26\/most-navy-c-130s-remain-grounded-nearly-one-year-after-fatal-crash\/\" target=_blank>halted all C-130 blade overhauls in September 2017<\/a>, when investigations first revealed the root of the July mishap. It started working with the other parties in the independent review team on a single, streamlined overhaul process for all the services\u2019 blades, and the new process is currently under review, Kubenic said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Most likely, it will be validated in time to restart blade overhauls in January 2019, stated Wendy Varhegyi, a spokeswoman for Air Force Materiel Command.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The new 21-step process is \u201clargely similar\u201d for both the Air Force and Navy blades, with \u201ca very small number of differences due to service-unique propeller configurations and differing engineering assessments,\u201d Varhegyi said.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/airforcetimes\/news\/your-air-force\/2018\/02\/18\/work-on-navy-and-marine-c-130s-expected-to-bring-400-jobs-to-robins-air-force-base\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"203\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/6142305956_b41472b624_b.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Work on Navy and Marine C-130s expected to bring 400 jobs to Robins Air Force Base<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">Robins Air Force Base in central Georgia will soon begin doing overhaul maintenance on the C-130 airplanes flown by the U.S. Navy and Marine Corps.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">It makes a number of advancements to the processes used in 2011, such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Updating the fluorescent penetrant inspection, which involves submerging metal components into a fluorescent dye so that maintainers can see corrosion and cracks under a black light. Under the new process, the blades will be robotically submerged in the dye for a minimum of four hours, according to the investigation. This greater \u201cdwell time\u201d will aid in the &#8220;detection of potential corrosion or cracking of the propeller blades and critical components,\u201d Varhegyi said.<\/li><li>New eddy-current inspection equipment and procedures that automate some of the process, improve the sensitivity of the technologies used to find corrosion, and increase the area that is assessed.<\/li><li>A more advanced \u201ccomputer-controlled electronic borescope\u201d that captures imagery at higher definition and resolution than older models. \u201cThe new process will also be digitally recorded and retained for review by technicians or engineers if required,\u201d Varhegyi said.&nbsp;<\/li><li>Requiring that a \u201cglass bead blast\u201d is conducted on all blades before inspections. The process uses abrasive chemicals to clean metal surfaces and improves the application of protective coatings.<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In addition to these advances, WR-ALC is making the other changes recommended by the investigation, Kubinec said. It will now retain all maintenance documentation during a blade\u2019s lifecycle instead of disposing of records after a set period of time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">WR-ALC is revamping quality control and assurance procedures to ensure that all maintenance tasks are properly checked. (U.S. Air Force)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThis is a hard problem, and we\u2019re working very closely with the Navy and Marine Corps officials on it,\u201d Kubinec said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The WR-ALC commander could not quantify exactly how much the Air Force will spend on new equipment and training needed to stand up the new overhaul process, but stated that the sum was well worth it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe\u2019re getting the technology we need because we can\u2019t afford to let this happen again,\u201d Kubinec said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, the Marine Corps investigation warned that it may be more difficult than anticipated to implement the new process.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In an April memo, a Marine investigator applauded the independent review team\u2019s efforts to create a single set of procedures and rules for C-130 blade overhauls, stating that \u201cthe unique requirements in each publication for different branches of service is a critical issue that must be resolved immediately.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, the investigator expressed concern about the team\u2019s lack of chain of command, which he believed could muddy reporting requirements or implementation responsibilities.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The memo recommends that Naval Air Systems Command conducts a review to ensure Navy and Air Force are on the same page as they lay out the responsibilities and requirements of the new process.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It notes that the Navy still had not come up with a comprehensive evaluation process that ensures work is done correctly, while WR-ALC continued to lack the oversight needed to ensure compliance with technical documents. Most worryingly, WR-ALC &#8220;failed to show significant process improvements&#8221; even after the Navy&#8217;s initial audit after the mishap in August 2017.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;This is concerning considering the same procedural and quality control errors that allowed&#8221; the mishap propeller &#8220;and countless other propellers to erroneously enter the fleet could still exist,\u201d the memo states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigators were \u201chighly critical\u201d of the Air Force, and for good reason, said Pat Kumashiro, a retired Air Force colonel who served as the chief of the service\u2019s maintenance division from 2012 to 2016.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But unfortunately, as military aircraft get older, corrosion becomes more of a problem.
