The Navy announced the incident late Friday afternoon.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">\u201cCreating requirements without effective follow-through creates the illusion of procedural compliance, giving false comfort that leadership and supervisor understand duties and responsibilities,\u201d the investigation states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One involved sailor reported that he had seen his chief \u201cmaybe five times in two years.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cRegardless of which recommendations are ultimately adopted, the (Marine Corps Security Force Battalion Bangor) must align, streamline and simplify all associated directives, policies and programs,\u201d the investigation states. \u201cResponsibility and authority must be clearly codified and appropriate training and procedures established to prevent a similar outcome.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two of the three involved sailors received nonjudicial punishment, while two other sailors in the command \u201creceived other disciplinary actions,\u201d according to Strategic Weapons Facility Pacific spokeswoman Brandie Klaahsen.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Klaahsen did not answer emailed questions about what specific reforms have been implemented within the command following the sinking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cFollowing the investigation, a thorough evaluation was conducted, and appropriate corrective actions and disciplinary measures were taken,\u201d she said in a statement. \u201cA safety stand down was directed that emphasized professional knowledge, safety, risk mitigation and communication.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The boat crashed into the concrete buoy at 7:19 p.m. that night, and \u201ca thorough review\u201d of the three sailors\u2019 training qualifications showed they were invalid due to improper screenings and \u201cfinal qualification approval below the authorized level,\u201d the report states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One sailor recalled that while they were out on the perimeter sweep, he told the coxswain driving the boat to slow down before they crashed into the concrete buoy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI said something to the Coxswain about speed and hazard three times,\u201d the sailor recalled. \u201cThere was no reaction as we hit the buoy. I yelled at the Coxswain to make the MAYDAY call.\u201d<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/navytimes\/news\/your-navy\/2023\/02\/08\/navy-salvages-security-patrol-boat-that-sank-in-washington\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"225\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Photo-2-SWFPAC-HSB-2.JPG.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Navy salvages security patrol boat that sank in Washington<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">The ship was raised from roughly 160 feet underwater at Hood Canal.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">That sailor said the entire starboard side of the boat quickly went underwater and he had to do a combat side swim because of an arm injury.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That sailor noted he had had six chiefs since arriving at the command in 2020 and that those onboard did everything in their power to avoid the incident.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe have some Coxswains that are very aggressive drivers,\u201d the sailor told investigators. \u201cPort Operations will routinely call to the boats for excessive speed and doing tricks.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another sailor onboard estimated they were going 20 knots but were only supposed to go 10 knots in restricted visibility.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIt was foggy, or either it was foggy because there was a lot of residue on the window which we had to keep wiping off,\u201d that second sailor said. \u201cWe were going faster than we were supposed to.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That sailor said all they remember is waking up on the pier.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI have no memory of being pulled out of the water but remember my clothes being wet,\u201d the sailor said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The boat\u2019s driver that night recalled having to wipe the windows as they kept fogging up, and that the boat drifted closer to the security barrier.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cBy the time I saw the buoy, it was too late,\u201d the sailor recalled, adding that he did not assign the crew to be lookouts or watch the radar.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That sailor said he couldn\u2019t sleep for three days after the crash due to his injuries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI have not been told of guidance regarding speed at night,\u201d he said. \u201cI was trained by aggressive drivers.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As the crew abandoned ship, they were not wearing \u201cmustang\u201d suits for the cold water, but their flotation devices were on.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">He added that he had done hundreds of similar sweeps and that the concrete buoy\u2019s sway depends on the weather.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The investigator wrote that, while the crew complained of intermittent fog and low visibility, \u201cmultiple senior personnel and rescue crew\u201d said there was no inclement weather that night.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cThe statements of the three boat crews may have been an attempt to corroborate stories to lessen or affect the outcome of the investigation,\u201d the report states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Among the investigation\u2019s recommendations is one that crews be made aware of weather and water temperature reports so they can bring along required safety equipment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><i>Correction: an earlier version of this report misstated the number of sailors who received non-judicial punishment.
