{"id":56934,"date":"2020-02-04T19:46:29","date_gmt":"2020-02-04T19:46:29","guid":{"rendered":"https:\/\/one.sightlinemg.com\/navytimes\/uncategorized\/2020\/02\/04\/va-dod-recommended-ptsd-therapies-dont-help-many-military-patients-review-finds\/"},"modified":"2026-08-20T16:12:55","modified_gmt":"2026-08-20T16:12:55","slug":"va-dod-recommended-ptsd-therapies-dont-help-many-military-patients-review-finds","status":"publish","type":"post","link":"https:\/\/one.sightlinemg.com\/navytimes\/veterans\/2020\/02\/04\/va-dod-recommended-ptsd-therapies-dont-help-many-military-patients-review-finds\/","title":{"rendered":"VA, DoD recommended PTSD therapies don\u2019t help many military patients, review finds"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">The psychotherapy approaches considered by the Departments of Veterans Affairs and Defense to be front-line treatments for military-related PTSD don\u2019t work for up to two-thirds of patients, a new report published in JAMA Insights finds.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cognitive processing therapy, or CPT, and prolonged exposure therapy, PE, are two approaches for treating post-traumatic stress disorder that focus on a patient\u2019s traumatic experiences and helps them process the memories associated with the horrific events.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But a review of results from several large clinical studies conducted since 2015 on military personnel and veterans, researchers with New York University\u2019s Grossman School of Medicine found these psychotherapies have limited success in these patients, despite recommendation as<a href=\"https:\/\/www.healthquality.va.gov\/guidelines\/MH\/ptsd\/VADoDPTSDCPGFinal012418.pdf\" target=_blank> preferred treatments in the VA\/DoD Clinical Practice Guidelines for the Management of PTSD and Acute Stress Disorder<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In fact, according to the review published Jan. 30 by JAMA, interventions that don\u2019t focus on traumatic events, such as present-centered talk therapy, <a href=\"https:\/\/www.militarytimes.com\/2016\/05\/28\/transcendental-meditation-shows-promise-as-ptsd-therapy\/\" target=_blank>transcendental meditation<\/a> and medication, seem to work about as well as emotionally charged PE and CPT.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cOverall, these new findings suggest that first-line psychotherapies do not effectively treat military-related PTSD in large proportions of patients and do not outperform non-trauma-focused interventions,\u201d wrote lead author Maria Steenkamp, clinical assistant professor of psychiatry at Grossman.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report did find that all active treatments considered \u2014 including PE, CPT, present-centered therapy, meditation and medications \u2014 reduced PTSD symptoms, often \u201cwith moderate to large effect.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Still, just 31 percent to 50 percent of patients actually achieved what Steenkamp would call \u201ca clinical success.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe found that a third to half the patients respond well [to CBT or PE]. Of course that\u2019s the same way as saying two-thirds to half don\u2019t respond in a way that we would consider successful,\u201d Steenkamp said.<\/p>\n\n\n\t<aside class=\"smg-interstitial-link wp-block-smg-interstitial-link\">\n\t\t<a href=\"https:\/\/one.sightlinemg.com\/militarytimes\/management\/pay-benefits\/2019\/12\/02\/why-the-us-military-cant-recruit-more-mental-health-professionals\/\" class=\"smg-interstitial-link__inner\">\n\t\t\t\t\t\t\t<div class=\"smg-interstitial-link__media\">\n\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"180\" src=\"https:\/\/one.sightlinemg.com\/wp-content\/uploads\/2026\/08\/5735866.jpg.jpg?w=300\" class=\"smg-interstitial-link__image wp-post-image\" alt=\"\" \/>\t\t\t\t<\/div>\n\t\t\t\t\t\t<div class=\"smg-interstitial-link__content\">\n\t\t\t\t<span class=\"smg-interstitial-link__kicker\">Related<\/span>\n\t\t\t\t<h3 class=\"smg-interstitial-link__title\">Why the US military can\u2019t recruit more mental health professionals<\/h3>\n\t\t\t\t\t\t\t\t\t<p class=\"smg-interstitial-link__excerpt\">Low pay, fewer advancements and excessive workload are among the reasons mental health providers don&#039;t join or stay in the military.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/a>\n\t<\/aside>\n\t\n\n\n<p class=\"wp-block-paragraph\">Since at least 2010, DoD and VA have recommended trauma-based therapies for treating PTSD such as CPT, PE or eye-movement desensitization and reprocessing therapy, EMDR, as the standard of treatment over other interventions such as group therapy, family therapy, hypnosis and relaxation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In a literature review also published in JAMA in 2015, <a href=\"https:\/\/www.militarytimes.com\/pay-benefits\/military-benefits\/health-care\/2015\/08\/19\/efficacy-of-ptsd-treatments-questioned\/\" target=_blank>the NYU researchers questioned the effectiveness of the trauma-focused treatments<\/a>, given that nearly a quarter of patients who tried CPT or PE dropped out of therapy. They found that 70 percent of CPT or PE patient did experience symptom reduction, but two-thirds still met the criteria for having PTSD.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For the recent review, JAMA asked Steenkamp and the 2015 lead author, Dr. Charles Marmar, a psychiatrist at NYU Langone Medical Center, to update their work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cRecent PTSD treatment trials show that military-related PTSD is a complex and heterogenous condition, and treating it with a single course of trauma-focused monotherapy is not well tolerated by many patients and has limited efficacy,\u201d they concluded.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">According to VA, about 11 percent to 20 percent of post 9\/11 combat veterans and 12 percent of Gulf War veterans have PTSD in a given year. An estimated 30 percent of Vietnam veterans have had PTSD in their lifetimes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Steenkamp said the new research shows that VA and DoD providers should help their patients find treatments that work for them, rather than use a one-size-fits all approach.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As a practicing clinician, for example, she finds trauma-focused EMDR to be highly effective for many of her patients, she said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Her reviews findings have raised questions about DoD and VA policies that prioritize CPT and PE, she said, and she hopes they spark a broader conversation on new approaches to treating PTSD.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cStep 1 is to realize there needs to be some kind of change to the way the system is now.
